When you have a work-related injury at your place of business, what do you need to do?
For those with Society Insurance Company, call within 48 hours of the incident at 1 888 576 2438 to report the claim.
Please have as much of the following information available as possible:
- Company name
- Telephone number
- Name of contact person
- Workers compensation policy number
- Details of the work-related injury
- Name and contact info of the injured employee
- Wage info if there is a potential for lost time from work
If you prefer, you may fax a First Report of Injury form to 1 920 922 1071 or email to firstname.lastname@example.org.
Please report death claims via telephone or fax within 24 hours. Society prefers the claim be reported directly to them.
